Admission Application

Applying for admission into one of The Homestead’s programs is easy. The first step is completing this online application. Once the application is submitted, we’ll be in touch with further instructions and questions.

Although we cannot guarantee there will be space and staff available, we can guarantee that your application will be reviewed in a timely manner and that The Homestead team will be in contact shortly upon receipt.

The Homestead is the only Medicaid-approved provider of services specifically for people with autism and a child or adult must be on the Autism Spectrum (Autism, Aspergers, PDD-NOS) to be eligible for admission.

Please complete the following form in order to have your family member considered for entrance into one of The Homestead’s programs. You may want to have Speech Therapy plan, Occupational Therapy plan, Case Management information, SCL Provider/Goals, Respite Provider information, School IEP, Psychiatrist/Psychologist Evaluations available as you may need information from these documents as you complete this application.

Please note that due to the sensitive nature of the information, this application process will time out should you leave the computer idle for more than 30 minutes. (If the application times out, you will have to begin the process again as your information will not be saved.)

Upon completion of the online application, you will receive a confirmation email with contact information and further instructions.
